The purpose of a makiwara is train those areas you strike with so that they can strike harder without the same causing pain. Padding would defeat that purpose.
Traditionally, a makiwara is a wooden post, well set in the ground so that it is steady and unmoving. Certain areas are wrapped with roped to give it a coarseness that will help to toughen the skin. You practice until you can routinely strike it, using all striking areas of your body, will full force.
There is also a tradition of using larger, coarse stones that one strikes in a similar manner.
