As you can see from this, it varies from state to state, although there are some common trents, like blood tests, physicals, and eye exams.

As for Sonnen, he claims to suffer from hypogonadism, meaning his body is not producing enough testosterone. However, I've never seen him claim that he only had one testicle, or has heard that from anyone other than his opponents. I suspect they make that claim as a form of trash-talking. One Endocrinologist concluded that his hypogonadism is the result of having used testosterone since 2008- that is, he's used artificial testosterone for so long that his body had permanently decreased its own production:

I was watching an interview of a ufc fighter and he mentioned that Chael Sonnan has only one testicle but I didnt believe it because when I did boxing, we had to have a doctor sign a sheet of paper to say that we were fit to box. There were a few physical requirements to be deemed fit to box and one of them was that you had to have both balls. So Im just wondering if UFC has physical requirements and if not does Chael really have one ball?
